If you are completing this application on behalf of a patient, "you" and "your" refer to the patient and you confirm you are authorized to consent on their behalf.
What you should know about this consent
- You are being asked to provide consent for Family Reach to contact you about research opportunities related to your experience with our Financial Resource Center (FRC) services.
- This consent allows us to include your contact information in our research database and reach out to you about future studies examining patient experiences with financial assistance programs.
- Your participation in any specific research study will be completely voluntary, and you can decline any research invitation without affecting your current or future FRC services.

Purpose of research participation consent
Family Reach is committed to improving our services and understanding how financial assistance programs impact cancer patients. We conduct research to evaluate the effectiveness of our programs from the patient perspective and to identify areas for improvement. By providing this consent, you are allowing us to contact you about research opportunities that may help us better serve future cancer patients facing financial challenges.
Why you are being asked to participate
You are being asked to provide this consent because you are receiving or have applied for services through Family Reach's Financial Resource Center. Your experiences with financial challenges related to cancer treatment and with our assistance programs provide valuable insights that can help us improve services for other patients in similar situations.
